
Creatine for Women: 0.8 lb of Lean Mass in Nine Months. That Is the Bulk You Were Warned About.
You have been told creatine will make you bulky. So here is the number.
Across seven randomised trials and 608 women, creatine added 0.8 lb of lean mass (0.37 kg) — over a median of thirty-eight weeks. [1]
Nine months. Under a pound. That is the bulk.

The finding that is actually big
Lean mass is the number everybody worries about. It is not the number worth having.
On a leg press one-rep max — the heaviest load you can move once — creatine added 16.5 lb (7.5 kg). [1] Three trials, and they agreed with each other completely.
So the trade on offer is not “get bigger”. It is getting meaningfully stronger while the scale barely notices. Which is close to the opposite of the thing women are warned about.
It only worked with a dose and a stimulus
This is the sentence to take away, and it is the reviewers’ own: “Benefits were evident when creatine ≥ 5 g/day was combined with RT; trials using ≤ 3 g/day without RT showed no measurable effect.” [1] RT being resistance training.
Which should sound familiar if you read our protein verdict: the supplement is the material, the training is the stimulus, and without the second one the first does nothing measurable. Two different supplements, two different research groups, same conditional.
On bloating, 684 trials and a surprise
The other half of the warning is water — that you will puff up. There is a very large analysis of this: side effects reported across 684 randomised controlled trials and more than 12,800 people. [2]
Its finding on stomach complaints, kidney markers, liver markers and muscle complaints is that creatine did not raise them above placebo, even at high doses and long durations. And this line is worth reading twice: “placebo groups often reporting similar or greater SE frequencies.” [2] People taking nothing reported side effects at least as often.
One honest caveat the paper makes and its own title does not. Dose and duration were statistically associated with side-effect reporting — but “the effect sizes were uniformly small, events were infrequent, and the reported symptoms were primarily mild and nonspecific”, with no consistent pattern. [2] That is a more careful claim than “no association”, and it is the one the evidence supports.
Why this is Supported and not Established
Our main creatine verdict is rated Established. This one is a step below, and the reason is a piece of statistics almost nobody explains.
The meta-analysis reports two ranges for the lean mass result. The confidence interval — +0.11 to +1.52 lb — is about the average effect across these trials, and it does not include zero, so the average is real.
The prediction interval is different. It asks what the next trial would probably find. That one runs −0.22 to +1.85 lb [1] — and it does include zero. A new study finding nothing at all would not be surprising.
Seven trials, risk of bias mostly rated “some concerns”, with one large preregistered double-blind trial at low risk. [1] Good evidence. Not settled evidence.
Three limits worth having
These were postmenopausal women, mean age about 62. [1] That is who was studied, and we are not going to quietly stretch it to cover every woman. If you are thirty, this is the closest good evidence there is — not evidence about you.
Bone density did not move. [1] Creatine is increasingly marketed to older women on the bone argument. Across these trials it was unchanged, and the reviewers say the effect there remains unclear.
The field is small. These two papers share an author, which is unremarkable in a specialised area but worth a reader knowing. It is the same caution we apply anywhere a literature is produced by a tight-knit group. The review also notes it was not registered in advance.
The verdict
Supported. In postmenopausal women, at least 5 g a day alongside resistance training, creatine produced a small gain in lean mass and a considerably better one in leg strength, with side effects indistinguishable from placebo across 684 trials.
The bulk you were warned about is 0.8 lb over nine months. The strength is 16.5 lb on a leg press. Whoever told you to avoid it had those two the wrong way round.
